Position Overview:
The Clinician for NYAP's La Jornada (The Journey) Program focuses on Transitional Foster Care, striving to create a significant impact in the lives of minors who have arrived in the U.S. without a parent or legal guardian.
The Clinician collaborates closely with minors, foster families, sponsor families, and community partners, providing essential counseling, advocacy, and support to ensure successful integration into the community.
Key Responsibilities:
The Clinician will be responsible for:
- Upholding the mission, values, and philosophies of the National Youth Advocate Program in all tasks.
- Adhering to established intake, placement, and discharge protocols, including conducting and updating assessments and service plans.
- Documenting progress notes and ensuring timely uploads to the electronic health record (EHR) system.
- Conducting risk assessments and developing safety and crisis plans for minors.
- Engaging with foster parents and sponsor families to review all relevant documents and assessments.
- Providing individual, group, and family mental health services, ensuring established outcome thresholds are met.
- Traveling to various locations to deliver in-home services to minors and families.
- Coordinating community services, including medical, educational, and psychological support.
- Screening for human trafficking concerns and providing crisis intervention as needed.
- Assisting in placement efforts consistent with minors' case plans.
- Performing additional duties as assigned.
Minimum Qualifications:
• Master's degree in social work with clinical experience, licensed or eligible for licensure; OR a bachelor's degree with 5 years of clinical experience, licensed or eligible for licensure (where applicable).
• Flexibility to work non-traditional hours, including potential on-call responsibilities.
• Ability to organize and transport youth to various destinations, including travel arrangements.
• Valid driver's license and reliable vehicle required, with appropriate insurance coverage.
• Bilingual proficiency in English and Spanish is essential for serving the diverse needs of children and families.
• Proficient in using various technology tools, including computers and relevant software.
The Clinician is expected to embrace a team-oriented approach, support agency goals, and uphold the philosophy of caring for individuals and connecting with communities.
