Job SummaryWe are seeking a highly skilled Registered Nurse Case Manager to join our interdisciplinary team. As an RN Case Manager, you will play a vital role in coordinating patient care, ensuring seamless transitions between healthcare settings, and providing expert clinical guidance to patients and their families.
Key Responsibilities- Coordinate Patient Care: Develop and implement comprehensive care plans, ensuring patients receive the necessary levels of care to meet their unique needs.
- Interdisciplinary Collaboration: Work closely with our team of healthcare professionals, including physicians, social workers, and spiritual care coordinators, to ensure comprehensive patient care.
- Clinical Expertise: Provide expert clinical guidance to patients and their families, addressing their physical, emotional, and spiritual needs.
- Communication and Coordination: Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Documentation and Reporting: Maintain accurate and timely documentation of patient care, including progress notes, treatment plans, and other relevant information.
- Registered Nurse License: Current RN license in the state of employment.
- Experience: Minimum of one year of nursing experience, preferably in a Home Health/Hospice setting.
- Certifications: Certification in Hospice and Palliative Nursing is a plus, but not required.
- Skills and Abilities: Strong clinical, organizational, and interpersonal skills, with excellent written and oral communication skills.
This role requires the ability to work in an office and clinical environment, with occasional travel required. The incumbent must be able to lift up to 20 pounds and possess sufficient personal mobility and physical capability to perform the essential functions of the job.
