* Conduct home visits to assess patient needs and provide support
* Maintain accurate records and reports
* Ensure compliance with regulatory requirements

**Requirements:**

* Active driver's license and car insurance
* BLS and CPR certifications
* EMR system experience
* Ability to work independently and as part of a team
