We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN with Adoration Home Health, you will have the opportunity to grow your skillset while providing essential one-on-one care to individuals in their home setting.

**Responsibilities**

* Perform nursing procedures within the scope of LPN practice, including vital sign monitoring, medication administration, and patient teaching.

* Monitor patient intake and output, reporting significant changes immediately.

* Administer medications as instructed and document patient files accordingly.

* Participate in multi-disciplinary patient case conferences, staff meetings, and in-service education programs.

* Cooperate with on-call schedules, weekends, and after-hours visits as necessary.

**Qualifications**

* Licensed Practical Nurse/LPN license in good standing.

* Previous experience in home health, hospice, hospital, or skilled nursing preferred.

* Active CPR certification.

* Familiarity with electronic health records.

* Strong problem-solving and critical thinking skills.

* Ability to thrive in a fast-paced environment.

* Reliable transportation, valid driver's license, and current automobile insurance.
