About the Role

Bristol Hospice is seeking a highly skilled and compassionate Nurse Practitioner to join our team as a Hospice Care Specialist. As a key member of our interdisciplinary team, you will provide comprehensive hospice services to patients and their families, ensuring the highest level of care and compassion.

Key Responsibilities
  • Provide medical care and palliative management of terminal illness under the supervision of a collaborating physician
  • Conduct thorough assessments, develop individualized plans of care, and coordinate hospice services
  • Collaborate with the hospice interdisciplinary team to identify patient goals and needs
  • Communicate effectively with patients, families, and healthcare practitioners to ensure seamless care coordination
  • Participate in quality monitoring and improvement initiatives to ensure high-quality patient care
Requirements
  • Master's degree in Nursing with a minimum of one year of Nurse Practitioner experience and one year of hospice or palliative care experience
  • Current nursing licensure in the state and accreditation from an approved certifying body for advanced practice nursing
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment
  • Strong clinical knowledge and skills, with the ability to provide high-quality patient care
