Job Title: Licensed Practical Nurse

Caresense Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN, you will provide high-quality care to patients in their homes, working closely with our team to ensure the best possible outcomes.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, following physician orders and care plans.
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Administer medications and treatments as prescribed by physicians.
  • Monitor patient progress and report any changes or concerns to the healthcare team.
  • Collaborate with other healthcare professionals to ensure comprehensive care.
Requirements:
  • Current LPN licensure in the state of Pennsylvania.
  • 1 year of experience as an LPN in a home care setting.
  • Graduate of an accredited LPN program.
  • Proof of current CPR certification.
  • Ability to work a flexible schedule and travel locally.
