Responsibilities:
  • Patient Assessment: Conduct thorough and ongoing holistic nursing assessments to identify patient needs and concerns. Develop individualized plans of care that address these needs and prioritize interventions to achieve positive patient outcomes.
  • Patient Care/Discharge Planning: Collaborate with patients, families, and healthcare teams to coordinate care and ensure seamless transitions. Implement plans of care and facilitate interdisciplinary referrals as needed.
  • Teaching: Provide individualized, ongoing teaching to patients and families to promote health and wellness. Utilize evidence-based practice and critical thinking skills to process information and develop effective teaching plans.
  • Communication: Demonstrate mutual respect and collaboration when communicating with healthcare teams. Utilize effective communication skills, such as active listening and assertive communication, to convey a strong sense of advocacy and protect patient rights.
  • Treatment/Procedures: Perform treatments and procedures in accordance with established policies, procedures, and patient care standards. Collaborate with healthcare teams to ensure safe and effective care.
  • Documentation: Accurately and efficiently document patient information using the nursing process. Ensure that patient information is collected and communicated effectively to ensure excellent patient outcomes.
  • Medication Administration: Provide all aspects of the medication process, including assessment, administration, monitoring, and evaluation of patient response. Teach patients and families about safe medication use and initiate referrals to Pharmacy as needed.
  • Quality Performance and Improvement (QPI): Participate in QPI processes to achieve excellent patient outcomes and improve processes. Develop QPI plans, collect data, analyze results, and implement corrective action plans to sustain improvements.
  • Delegation to and Development of Co-workers: Accurately delegate patient care tasks to LPNs, CNAs, and other ancillary personnel according to their scope of practice, job description, individual competence, and patient needs. Supervise the provision of care and provide direction and supervision as needed.
Requirements:
  • Graduate of an accredited school of nursing, currently licensed in the State of New York, or permittee.
  • Minimum of one year Med/Surg experience preferred. Minimum of 2 years critical care or emergency room experience preferred.
  • Basic knowledge of nursing theory and practice. Knowledge of appropriate technology and practice standards for area of responsibility.
Training/License:
  • New York State Registered Nurse license required, or permittee.
  • BLS, ACLS, and PALS certifications required. TNCC, ENPC, and CEN preferred.
