Key Responsibilities:
  • Provide comprehensive psychosocial evaluations and counseling to patients and their families.
  • Develop and implement individualized plans of care to address patients' social and emotional needs.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
Qualifications:
  • Master's degree in Social Work from an accredited program.
  • Eligibility for membership in the National Association of Social Workers.
  • At least one year of experience as a Medical Social Worker in a medical or clinical setting, preferably in dialysis or with a physically ill population.
  • Licensure in Alaska or willingness to obtain licensure.
