Responsibilities:- Provide compassionate patient care in accordance with Professional Standards of Practice, abiding physician orders, federal, state, and local guidelines.
- Effectively communicate verbally and in documentation with all healthcare team members.
- Record patients' medical histories, symptoms, and treatment plans.
- Perform nursing assessments and collaborate with staff to develop effective plans of care.
- Establish realistic, measurable short and long-term patient goals with interventions put in place as needs arise to minimize rehospitalizations and continued evaluations of outcomes.
- Assist staff with answering patient call lights, answering telephones, passing meal trays, and transporting residents.
- Identify and support patient and family needs for health education.
- Follow up with appropriate teaching strategies.
- Prepare and administer patients' medications and treatments per healthcare provider's orders and in accordance with federal, state, local, and facility policies and procedures.
- Monitor and record changes in patient physical and/or mental status, signs & symptoms of condition recovery, deterioration, or adverse effects.
- Communicate and delegate patient care to assigned staff.
- Supervise, ensuring implementation of tasks and interventions.
- Care for patients, staff, and self by supporting and promoting a culture of safety in the workplace and complying with Infection Control Procedures.
- Recognize hazards, report, and investigate incidents in a timely manner in accordance with facility policies.
- Strong clinical skills and ability to react decisively in emergency situations.
- Verbal and written communication skills.
- Valid Registered Nurse (RN) license.
- Valid CPR/BLS certification.
- Current TB testing documentation.
- Current physical examination.
