We are seeking a highly motivated and experienced Registered Nurse Case Manager to join our team at Focus Staff. As a Registered Nurse Case Manager, you will be responsible for providing case management services to patients according to policy and procedure.
Responsibilities:- Provide case management services to patients, including assessment, planning, implementation, and evaluation of care.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure understanding and compliance with care plans.
- Current state license as a Registered Nurse.
- Minimum of three years of hospital-based nursing experience, with a focus on utilization/case management.
- BLS for Healthcare Providers certification required within 30 days of hire.
