You will work closely with members to manage their health and healthcare, utilizing strong communication and interpersonal skills to build trust and rapport.

Key Responsibilities:

  • Assess and plan care strategies for members with complex medical and behavioral needs
  • Implement care plans and provide ongoing support to members
  • Communicate with members, families, and healthcare providers to ensure seamless care coordination
  • Identify and initiate referrals for social services and community resources
  • Collaborate with interdisciplinary teams to ensure appropriate services and identify needs

Requirements:

  • Current, unrestricted RN license in the state of Kansas
  • 2+ years of experience working in the community as a RN in acute inpatient care, long-term care, home health, hospice, public health, behavioral health, or assisted living
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook
  • Ability to travel up to 75% of the time with reliable transportation, with a current and non-restricted state of Kansas Driver's License and State-required insurance

Preferred Qualifications:

  • Bachelor's degree in nursing
  • Background in managing Medicaid and Medicare populations with complex medical and behavioral needs
  • Experience with arranging community resources
  • Experience with electronic charting
