The Patient Care Coordinator is a vital member of our team at Tucson Physician Group Holdings, responsible for providing exceptional patient care and support in a fast-paced physician office environment.
Key Responsibilities:
- Assess patient needs and develop treatment plans in collaboration with physicians, taking into account vital signs, medical history, and medication lists.
- Perform various tests and procedures as directed by physicians, ensuring accurate and timely results.
- Prepare equipment and assist physicians with medical treatments, exams, and procedures, maintaining a safe and sterile environment.
- Draw lab specimens and prepare them for lab transport, accurately completing lab forms and computer entries.
- Administer prescribed medications and injections, ensuring patient safety and comfort.
- Provide patient education and instructions, promoting health literacy and self-management.
- Triage patients, identifying emergencies and providing immediate support as needed, notifying physicians of critical situations.
- Contribute to the development and documentation of patient care protocols, procedures, and education materials, ensuring consistency and quality.
- Analyze medical, statistical, and operational data to prepare reports on outcomes and program efficacy, informing business decisions.
- Verify treatment forms for accurate data entry, maximizing reimbursement and streamlining processes.
- Ensure accurate and timely entry of patient information into the Electronic Health Record, maintaining data integrity.
Requirements:
- High school diploma/GED and completion of an accredited nursing program.
- Current State RN nursing license and CPR certification.
- Minimum of two years of clinical experience working in a physician office.
