Pharmacist - Clinical and Production Services

Penn Medicine is seeking a highly skilled Pharmacist to join our team. As a key member of our healthcare team, you will be responsible for providing clinical and production pharmacy services to assigned patients. This role requires strong clinical decision-making skills, attention to detail, and excellent communication skills.
Key Responsibilities- Manage clinical and production pharmacy services, including checking filled orders and supplies, performing sterile compounding as needed, and overseeing the work of pharmacy technicians and other pharmacists.
- Provide clinical decisions regarding patient-specific pharmaceutical care, including assessment of patient needs, review of medication profiles, evaluation of duplicate therapies, and development of patient databases and documentation in medical records.
- Accept physician prescription orders and evaluate their appropriateness, including dosage, drug choice, potential drug interactions, and route of administration.
- Participate in the development of multi-disciplinary plans of care, establishment of goals and objectives of prescribed therapy, and recommendations for efficient and effective dose administration.
- Monitor patient response to treatment, including subjective and objective assessment, laboratory tests, and discussion of results with nurses and physicians.
- Provide recommendations to improve or adjust therapy based on lab evaluations and patient-specific drug information to patients, caregivers, prescribing physicians, and other healthcare professionals.
- Communicate effectively and document actions with physicians, nurses, and patients in software databases, ensuring complete and accurate patient information.
- Ensure compliance with state and federal regulations, as well as JCAHO standards, by maintaining required documentation, including prescription logs, patient medication profiles, patient charts, narcotic receipts, and state pharmacy permits.
- Pharmacist licensure in the State of Pennsylvania.
- Bachelor of Pharmacy degree from an accredited College of Pharmacy.
- Less than 1 year of pharmacist experience required.
