Key Responsibilities:
  • Provide comprehensive, skilled nursing care to patients in their homes, focusing on their unique needs and goals.
  • Collaborate with our interdisciplinary team to develop and implement personalized care plans that promote optimal health and well-being.
  • Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions to enhance patient outcomes.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and transitions.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices to provide the highest quality care.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum of one year of nursing experience, preferably in home health or a related setting
  • Strong med-surg, ICU, ER, or acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Excellent communication, organizational, and time management skills
  • Valid driver's license, auto insurance, and reliable transportation
