Quality LPN Medical Records Specialist

The LPN Medical Records Specialist will be responsible for providing high-quality patient care and service in a clinical setting. This role requires proficiency in all tasks performed by clinical coworker staff, as well as acting as a role model, trainer, and mentor for clinic staff.
Key Responsibilities:
• Assists with gathering and delivering information to team members in accordance with the patient-centered medical home huddles.
• Reviews patient charts and identifies gaps in patient care in accordance with the patient-centered medical home model.
• Determines work priorities and assists, as needed, to ensure timely work flow for patient care.
• Assists in clinic with patient care as needed, including rooming patients, medication refills, referrals, and phone calls.
• Completes medical prior authorization, as needed.
• Triage patients in person and via telephone.
• Supports patient care delivery by helping healthcare providers during examinations, preparing laboratory specimens, and performing basic laboratory tests on the premises.
• Assumes self-responsibility for all direct patient care activities, as indicated by the Registered Nurse (RN), within the scope of the LPN license.
Requirements:
• 1 year clinic experience preferred.
• Computer skills required.
• Electronic Medical Record experience preferred.
• Dedicated to quality and accuracy.
• Self-starter and able to work with minimal supervision.
• Ability to maintain high level of confidentiality.
