Job Summary:

The Care Manager I is a vital member of our healthcare team, responsible for ensuring seamless patient progression through the continuum of care. This role requires collaboration with patients, families, physicians, and multidisciplinary team members to develop and implement effective care plans. The ideal candidate will possess excellent communication and analytical skills, with the ability to work independently and exercise sound judgment in a fast-paced environment.

Key Responsibilities:

  • Conduct thorough assessments of patient needs, identifying social, emotional, and financial factors that impact health status.
  • Develop and monitor patient care plans, ensuring effectiveness and appropriateness of services.
  • Coordinate patient care progression throughout the continuum of care, promoting efficient and cost-effective outcomes.
  • Serve as a resource and advocate for patients, providing support and guidance on treatment decisions and end-of-life issues.
  • Closely monitor patient length of stay, collaborating with interdisciplinary team members to remove barriers and expedite discharge.
  • Identify and escalate local and system barriers impeding diagnostic or treatment progress, ensuring timely resolution.
  • Work to resolve identified delays to discharge, collaborating with medical staff, nursing staff, and ancillary staff to eliminate barriers to efficient care delivery.
  • Assess needs for discharge planning and continuing care/resource support, making recommendations to patients and families regarding post-acute level of care needs and options.
  • Ensure appropriate communication and updates are provided to patients, families, and healthcare team members, documenting as necessary to assure continuity of care.
  • Provide interventions demonstrating knowledge of and sensitivity toward cultural diversity, religious, developmental, health literacy, and educational backgrounds of the patient population.
  • Collaborate with physicians and other healthcare professionals to promote appropriate use of medical center resources.
  • Actively participate in multidisciplinary/patient care progression rounds, escalating cases as appropriate and per policy to Physician Advisors and/or CMO Director.
  • Document in the medical record per regulatory and department guidelines.

Requirements:

  • Graduate of an accredited school of nursing or Bachelor's Degree in Social Work (BSW) required.
  • Experience in the clinical or acute care setting preferred.
  • LVN/LPN or LBSW in the state of employment is required.
