We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Case Manager. As a key member of our care team, you will be responsible for providing high-quality patient care and support to our clients in the comfort of their own homes.

Responsibilities
  • Conduct thorough patient assessments and develop personalized care plans to meet their unique needs.
  • Collaborate with the care team to ensure seamless coordination of care and services.
  • Provide education and support to patients and their families to promote health and independence.
  • Supervise and mentor Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure high-quality care delivery.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Communicate effectively with the clinical manager and care team to ensure optimal patient care.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
  • One (1+) year of clinical experience as a Registered Nurse (RN).
  • Current RN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
