Job Summary

We are seeking a compassionate and motivated Registered Nurse - RN with a wound focus to integrate and lead a patient-centered team approach for skin health and wound care as a Wound Care Coordinator - RN at Albuquerque Heights Healthcare and Rehab in Albuquerque, NM.

Key Responsibilities
  • Lead and integrate a fully developed Skin Health and Wound management program in collaboration with nursing center management.
  • Oversee durable medical equipment related to the prevention and management of wounds.
  • Develop and implement wound care protocols and procedures to ensure high-quality patient care.
  • Collaborate with interdisciplinary teams to provide comprehensive wound care services.
  • Conduct wound assessments and develop individualized care plans to promote wound healing and prevent further complications.
  • Provide education and training to nursing staff on wound care best practices and protocols.
  • Participate in quality improvement initiatives to enhance wound care services and patient outcomes.
Requirements
  • Must possess a RN license.
  • Must complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire.
  • Commitment to attend monthly support calls is required.
  • Must have national accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire.
  • Clinical experience in a skilled nursing setting is strongly preferred.
  • Discharge planning experience is strongly preferred.
  • Must have a strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team.
  • Experience in team building and demonstrating respect for the interprofessional team in wound management.
  • Knowledge of nursing home regulations and survey process is required.
  • Strong Microsoft Word, Excel, Database Management, PowerPoint skills are required.
