Position Overview: The Rehabilitation Specialist is tasked with executing care standards for rehabilitation services and ensuring adherence to all stipulations outlined in the Service Agreement.
Key Responsibilities:
- Assess patients' functional abilities and home settings to determine needs and formulate a comprehensive care strategy.
- Conduct work capacity assessments for patients re-entering the workforce and may offer specialized vocational training.
- Create treatment plans, establish objectives for enhanced functionality, and maintain effective communication with referring physicians and healthcare teams.
- Provide training in daily living activities, design splints, and guide patients in utilizing adaptive devices.
- Oversee Certified Occupational Therapy Assistants and engage in discharge planning.
- Keep precise clinical documentation and relay any changes in the patient's condition to the physician.
- Work in conjunction with other healthcare professionals to ensure cohesive care and participate in necessary meetings and discussions.
- Recognize patient and family requirements for supplementary home health services and facilitate appropriate referrals.
Qualifications:
- Graduated from an accredited rehabilitation therapy program.
- Registered with the National Registration Examination.
- A minimum of two years of clinical experience as a Rehabilitation Specialist.
- CPR Certification and a valid driver's license with insurance.
Required Skills:
- Exhibits outstanding verbal and written communication abilities, along with strong organizational skills.
