Job Summary:

As a Hospice RN with HexaQuEST Health, Inc., you will play a vital role in providing high-quality care to patients and their families. This is a challenging yet rewarding position that requires strong nursing skills, compassion, and a commitment to delivering exceptional patient outcomes.

Key Responsibilities:

• Assess, delegate, coordinate, and evaluate the nursing care provided to patients

• Develop and implement individualized care plans that meet the unique needs of each patient

• Collaborate with the interdisciplinary team to ensure seamless care coordination and communication

• Provide education and support to patients and their families to promote self-care and independence

• Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

• Current active RN license in the state of practice

• Minimum 2 years of experience in hospice care or a related field

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Familiarity with electronic medical records (EMRs) and other healthcare software
