Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Home Healthcare Specialist. As an LPN with Ambercare, you will play a vital role in providing direct patient care and implementing care plans in the comfort of patients' homes.

Key Responsibilities
  • Direct Patient Care: Provide skilled nursing care to patients in their homes, as defined by the state Nurse Practice Act.
  • Plan of Care Implementation: Implement care plans initiated by registered nurses, ensuring accurate and timely documentation.
  • Patient Education: Assess and provide education to patients and their caregivers on self-care techniques, diagnosis, and care plans.
  • Interdisciplinary Collaboration: Participate in the coordination of home health services, reporting identified needs to registered nurses and clinical supervisors.
  • Equipment and Supply Management: Use and prepare equipment and supplies effectively and efficiently, adhering to aseptic technique.
  • Professional Growth and Development: Participate in personal and professional growth and development opportunities.
  • Additional Duties: Perform other duties as assigned by registered nurses.
Requirements
  • Education: Graduate of an accredited practical nurse or vocational nursing program.
  • Experience: Three years of nursing experience, with community/home health or medical/surgical experience preferred.
  • Licensure: Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
  • Certification: Possess and maintain valid CPR certification.
  • Transportation: Must have reliable transportation, a current driver's license, and appropriate automobile insurance.
