About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our interdisciplinary team at Canyon Transitional Rehab Center in Albuquerque, NM. As a LPN, you will play a vital role in delivering high-quality patient care and achieving positive clinical outcomes.

Key Responsibilities
  • Deliver efficient and effective nursing care to patients, ensuring their physical and emotional needs are met.
  • Evaluate patients to collect data, observe condition, and report changes in condition to the healthcare team.
  • Contribute to nursing assessments and care planning, working collaboratively with the interdisciplinary team to achieve optimal patient outcomes.
  • Administer medications and perform treatments as ordered by physicians, ensuring patient safety and comfort.
  • Supervise and monitor patient care provided by unlicensed staff, ensuring high-quality care is delivered at all times.

Qualifications
  • Must be a graduate of an approved school for practical or vocational nursing.
  • Must be currently licensed by the State Board of Nursing or pending licensed practical nurse licensure.
  • Experience in rehabilitative or geriatric nursing is preferred.
  • Must maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility.
