Covenant Health Inc. is seeking a skilled Clinical Care Coordinator to join our team. As a key member of our clinical staff, you will provide high-quality patient care and support to our patients and families.
Key Responsibilities- Assess patient's general condition and take vital signs and weight
- Assist provider with examinations, diagnostic procedures, and treatments
- Administer patient medications per provider order and policy
- Orient, instruct, and train assigned personnel as applicable
- Provide basic patient care as ordered
- Assure test results are received and relayed to patient in a timely manner
- Document all interventions, patient/family responses, medication dispensed/prescribed, etc., in the medical record
- Promote wellness by providing patient education materials and communicating provider's advice and instructions
- Triage patient phone calls and urgency of appointments
- Ensure an adequate stock of supplies and well-maintained equipment
- Monitor and ensure accuracy of recording on patient medical records
- Minimum Education: None specified
- Minimum Experience: One (1) year of experience required as an LPN or as a certified Medical Assistant from a recognized certifying organization
- Licensure Requirement: Must be licensed as an LPN in the state of Tennessee or be certified as a Medical Assistant from a recognized certifying organization
