Weekend Hospice Registered Nurse

Amedisys is seeking a skilled Registered Nurse to join our team as a Weekend Hospice On-Call RN. This role requires a compassionate and dedicated individual to provide high-quality patient care on weekends, responding to telephone requests and making in-person visits as needed.
Responsibilities- Perform initial nursing assessments on new admissions
- Develop, coordinate, and revise individualized patient care plans
- Regularly communicate patient progress to the clinical manager, physician, and care team
- Record pain/symptom management changes/outcomes as appropriate
- Educate families on patient condition, needs, and care
- Make referrals to other hospice care disciplines as needed
- Perform on-call responsibilities and services to patients/families as assigned
- Current RN license, specific to the state(s) assigned to work
- One+ year of nursing experience required
- Current CPR certification
- Valid driver's license, reliable transportation, and liability insurance
