Part-time Home Health Nurse

As a Per Diem Registered Nurse Case Manager, you will play a vital role in our team, working closely with patients, families, and healthcare professionals to provide comprehensive care.
Key Responsibilities:- Conduct thorough assessments of patients to determine their home care needs, including Start of Care (SOC) Admissions and follow-up visits.
- Develop and implement personalized care plans, collaborating with physicians and agency staff to ensure the best possible outcomes.
- Monitor patients' conditions, reporting changes to the clinical team and making adjustments to care plans as needed.
- Accurately document observations, interventions, and evaluations, maintaining accurate and up-to-date records.
- We offer a flexible schedule, allowing you to create your own hours and work at your own pace.
- Our dedicated resources assist with administrative tasks, giving you more time to focus on patient care.
- Targeted territories ensure that patient visits are typically within a 5-15 mile radius, reducing travel time and increasing efficiency.

- Current and unrestricted CA RN license.
- Current BLS/CPR Certification.
- Valid CA Driver's License.

- High level of clinical knowledge in home health care is highly preferred.
