Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare in Highland Park, New Jersey. As an LPN, you will provide compassionate nursing care to patients in post-acute facilities, monitoring their physical and mental conditions and tracking their vital signs.

Key Responsibilities
  • Provide nursing care to patients in post-acute facilities
  • Monitor patients' physical and mental conditions and track their vital signs
  • Perform resident intakes, document medical histories, and track health concerns
  • Administer and verify medications, track use and effects
  • Cultivate a comfortable and safe facility for residents
  • Track medical records and charts
  • Assist with IV setup, immunizations, and phlebotomy procedures
  • Maintain medical supplies and inventory
  • Support staff in orientation and training of new colleagues
Requirements
  • Current New Jersey state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved LPN educational program
  • High school diploma (or GED/equivalent)
  • At least 1 year of experience (LPN or related role)
