SalaryThe estimated annual salary for this role is $32,455 - $56,433, depending on location, job-related knowledge, skills, and experience.
Job DescriptionOverview: We invite skilled medical social workers to join our community-based care centers with a supportive and inclusive work environment. This role offers better work/life balance and increased flexibility compared to other settings, as well as job stability and opportunities to advance with a growing company.
Responsibilities:
- Assists patients and families with health-related financial, social, and emotional concerns and assists with effective coping skills.
- Provides social work services in accordance with the plan of care.
- Educates patients and family members on medical social work interventions and treatment plans.
- Communicates identification of risks or threats to patients to physicians and other healthcare team members to coordinate care effectively.
Required Skills and Qualifications:
- Holds a master's or doctorate degree from a school of social work accredited by the Council on Social Work Education.
- Has one year of social work experience in a healthcare setting.
- Meets state and federal requirements for this position.
- Current CPR certification.
