Responsibilities- Assist patients and families with health-related financial, social, and emotional concerns, and help develop effective coping skills.
- Provide social work services according to the plan of care, ensuring timely communication with healthcare team members and office staff.
- Educate patients, family members, and team members on medical social work interventions and treatment plans.
- Perform care planning, transfer, and discharge functions, identifying and mitigating risks or threats to patient health.
- Masters or doctorate degree from a school of social work accredited by the Council on Social Work Education.
- At least one year of social work experience in a healthcare setting.
- Meets state and federal requirements for this position.
- Current CPR certification.
