Job SummaryWe are seeking an Advanced Practice Provider to join our team in Primary Care. As a member of our team, you will provide high-quality patient care, work collaboratively with our healthcare team, and contribute to the development of our healthcare services.
Responsibilities- Provide primary care services to patients, including health assessments, diagnosis, and treatment.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with our healthcare team to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Stay up-to-date with the latest medical research, technologies, and best practices to provide evidence-based care.
- Master's Degree in Nursing as Nurse Practitioner, from a nationally accredited postgraduate certificate program required.
- Registered Nurse Practitioner Licensure for; and listed on the state Advanced Practice Nurse (APN) Registry required.
- Board certified as an Advanced Practice Registered Nurse, in applicable population foci and/or subspecialty.
- 1 year of experience in a clinical or advance practice setting, preferred.
