Job Title: Individual Support Coordinator

Estimated Salary: $50,000 per year

Job Overview:

We are seeking an experienced and compassionate Individual Support Coordinator to join our team at the Pearl Buck Center. As an Individual Support Coordinator, you will play a critical role in supporting individuals with intellectual or developmental disabilities to achieve their goals and live fulfilling lives.

Key Responsibilities:

  • Conduct thorough assessments and develop Individualized Support Plans (ISPs) that meet the unique needs of each individual.
  • Collaborate with multidisciplinary teams to ensure comprehensive support and services for individuals.
  • Coordinate and facilitate community-based programs and activities to promote independence and socialization.
  • Maintain accurate records and documentation of individual progress and outcomes.
  • Communicate effectively with families, caregivers, and other stakeholders to ensure seamless support and services.

Requirements:

  • Bachelor's degree in a related field, such as special education, social work, or healthcare.
  • 2-3 years of experience working with individuals with intellectual or developmental disabilities.
  • Valid Oregon driver's license and reliable transportation.
  • Ability to pass a background check and maintain confidentiality.
