Nurse Practitioner Role

We are seeking an experienced Nurse Practitioner to provide high-quality patient care and collaborate with our interdisciplinary care team at HealthCare Support in Eugene, Oregon.

As a key member of our team, you will be responsible for delivering comprehensive assessments, developing effective treatment plans, and managing acute and chronic medical conditions. Your expertise in advanced illness and end-of-life discussions will enable you to provide valuable support to patients and their caregivers.

Key Responsibilities:
  • Conduct home visits to up to 20-30 locations throughout Eugene or Lane County, including private homes and communities.
  • Develop and implement personalized treatment plans based on thorough patient assessments.
  • Manage complex medical cases, ensuring accurate and complete documentation.
  • Communicate effectively with the interdisciplinary care team to ensure seamless coordination of care.
  • Provide counseling and education to patients and their families, empowering them to manage their health effectively.
Requirements:
  • Active and unrestricted NP/PA license in Oregon.
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Minimum 1 year of experience as a Nurse Practitioner.
  • Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, Adult-Gerontology Acute Care Nurse Practitioner.
Preferred Qualifications:
  • Experience in long-term care settings.
  • Familiarity with geriatrics and chronic illness management.
  • Understanding of advanced illness and end-of-life discussions.
  • Strong computer skills, including proficiency in electronic medical records.
