The state-supported living centers cater to individuals with intellectual and developmental disabilities who may be medically fragile or face behavioral challenges.

These centers deliver campus-based direct services and support across 13 locations statewide.

**Key Responsibilities:**
- Perform routine nursing tasks, which include assessing, caring for, and treating individuals.
- May provide training to others.
- Operate under moderate supervision, with limited discretion for independent judgment.
- Work under the guidance of the Nurse Manager.

The Direct Care RN I may delegate nursing tasks to the Direct Care LVN in accordance with the Nurse Practice Act.

The Direct Care RN is responsible for overseeing and delivering professional nursing care to individuals residing in a State Supported Living Center.

The Direct Care RN is available to unit staff for nursing decisions around the clock.

As a member of the professional team, the RN shares responsibility with colleagues for developing individual care programs.

This role requires effective communication skills, including:
- Sharing information with team members, Primary Care Providers (PCP), and nurses;
- Data entry and documentation;
- Completing reports and maintaining personnel records;
- Supervisory duties while adhering to all facility rules and policies. Provide treatment to HHS employees injured during the course of their employment.

Essential Job Functions:

- Maintain regular attendance and may be required to work a specific shift schedule, including rotating shifts and/or overtime as per agency leave policy, while performing additional assigned duties.

- Administer medications, immunizations, and treatments while monitoring individuals for unusual symptoms and reactions.

- Conduct physical assessments to identify individual health needs and take appropriate actions.

- Implement and oversee case management service plans.

- Instruct and assist individuals in addressing their healthcare needs.

- Prepare, maintain, and review individual records and assessments.

- Participate in quality assurance reviews as needed.

- Comply with all State/Federal/Local standards, Facility Policy, ICF Standards, and the Texas Nurse Practice Act.

- Manage individual medical records (both paper and electronic).

- Provide treatment to HHS employees injured during the course of their employment.

Additional responsibilities may include actively participating in or supporting the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation, which may require alternate shift assignments and/or locations.


Knowledge, Skills, and Abilities:

- Familiarity with nursing techniques and procedures, healthcare laws and regulations, medical diagnoses and treatments, and community health principles.

- Ability to deliver individualized care, maintain accurate records, explain public health laws, and communicate effectively.

Must maintain current CPR certification and possess the strength and flexibility to perform Cardiopulmonary Resuscitation (CPR) safely.
Must demonstrate a solid understanding of emergency procedures and equipment.
