The state-supported living centers cater to individuals with intellectual and developmental disabilities who may be medically fragile or exhibit behavioral challenges.

These centers deliver campus-based direct services and support across 13 locations statewide.

Responsibilities include performing routine nursing tasks. Duties involve assessing, caring for, and treating individuals. May provide training to others. Operates under moderate supervision, with limited latitude for independent decision-making. Works under the guidance of the Nurse Manager.

The Direct Care RN I may delegate nursing responsibilities to the Direct Care LVN in accordance with the Nurse Practice Act.

The Direct Care RN is responsible for overseeing and providing professional nursing care to individuals residing in a State Supported Living Center.

The Direct Care RN is available to unit staff for nursing decisions around the clock.

As a member of the professional team, they share responsibility with colleagues for developing individual care programs.

Work entails effective communication skills, including

relaying information to team members, Primary Care Providers (PCP), and nurses; data entry, documentation, report completion, maintaining personnel records, supervisory duties, and adherence to facility policies.

Provides treatment to employees injured during the course of their work.

Key Responsibilities:

Regular attendance is required, and you may be expected to work a specific shift schedule or, at times, a rotating schedule, extended shifts, and/or overtime in line with agency leave policies while performing other assigned duties.

Administers medications, immunizations, and treatments while monitoring individuals for unusual symptoms and reactions.

Conducts comprehensive assessments of individual health needs and takes appropriate actions.

Implements and oversees case management service plans.

Guides and assists individuals in addressing their healthcare needs.

Prepares, maintains, and reviews individual records and assessments.

May engage in quality assurance evaluations.

Adheres to all State/Federal/Local standards; Facility Policies, ICF Standards, and the Texas Nurse Practice Act.

Manages individual medical records (both paper and electronic).

Provides treatment to employees injured in the course of their work.

Additional duties may include actively participating and/or serving in a supportive role to fulfill the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.

Such participation may necessitate an alternate shift pattern assignment and/or location.

Required Knowledge, Skills, and Abilities:

Familiarity with nursing techniques and procedures; health care laws and regulations; medical diagnoses and procedures; accepted medical treatment programs; and principles, practices, and procedures of community health and nursing care.

Ability to provide individualized care, maintain records, explain public health laws, and communicate effectively.

Must hold current CPR certification.
Must possess the strength and flexibility to perform Cardiopulmonary Resuscitation (CPR) safely.
Must demonstrate a working knowledge of emergency procedures and equipment.
