Job Summary:
As a Hospice Liaison at LifeBridge Health, you will play a vital role in ensuring that all services are delivered in accordance with the organization's Mission Statement and S.P.I.R.I.T. Values. Your primary responsibility will be to serve as a consultant for Carroll Hospice, acting as an advocate for Carroll Hospice programs and providing education for healthcare professionals, patients, and families in the surrounding community.
The purpose of this role is to improve care for all patients who have a life-limiting illness and specifically to facilitate admission of eligible patients to hospice care in the setting of their choice.
Key Responsibilities:
- Collaborate with healthcare professionals, patients, and families to provide education and support related to hospice care.
- Act as a liaison between Carroll Hospice and healthcare providers to ensure seamless transitions and coordination of care.
- Develop and maintain relationships with community partners to promote hospice care and services.
- Conduct assessments and evaluations to identify patients who may benefit from hospice care.
- Develop and implement plans to facilitate admission of eligible patients to hospice care.
Requirements:
- Associate's degree in Nursing required, BSN preferred.
- 2-4 years of RN experience in hospice care, regulations, management, administration, marketing, education, or discharge planning required.
- Ability to collaborate with others and to initiate and plan own work activities.
- Effective patient and staff teaching.
- Maryland Registered Nurse License, American Heart Association CPR Healthcare Provider Certification.
