We are seeking a skilled Nurse Care Coordinator to join our team at Remarkable Hospice Dublin location. As a key member of our interdisciplinary team, you will provide high-quality patient care and services to in-home clients.
Responsibilities:- Assess and provide hands-on care to patients
- Admit and discharge patients after hours, including death pronouncement
- Communicate with the interdisciplinary team and document necessary information in the electronic healthcare record
- Collaborate with the hospice physician and coordinate necessary ancillary services
- Provide services collaboratively with other healthcare providers and oversee unlicensed assistive personnel
- Offer education and support to family, caregivers, and facility staff
- Current RN License in good standing
- Minimum of 1-year nursing experience, with hospice experience a plus
- Current CPR certification and proof of current auto insurance
- Strong organizational and communication skills
Benefits: 15 paid days off plus 8 holidays, 401k with excellent match, comprehensive medical, dental, and vision insurance plans, mileage reimbursement, and a sign-on bonus of $500.
