Senior LTSS Service Care Manager RN

We are seeking a highly skilled and compassionate RN Care Manager to join our team at Centene Corporation. As a Care Manager, you will play a critical role in assessing and coordinating medical and supporting services for complex and high-acuity populations. Your expertise will help us deliver high-quality, cost-effective care to our members.
Key Responsibilities- Evaluate the service needs of complex and high-risk members and develop personalized care plans to promote quality and cost-effective care.
- Collaborate with healthcare providers, community resources, and family members to ensure members receive adequate and appropriate person-centered care.
- Monitor member status, change in condition, and progress towards care plan goals, and revise or update care plans as necessary.
- Collect, document, and maintain all member information and care management activities to ensure compliance with state and federal regulations.
- Provide education and support to members and their families on disease processes, healthcare provider instructions, care options, referrals, and healthcare benefits.
- Act as a liaison and member advocate between members, physicians, and facilities/agencies.
- Coordinate community resources and provide coordination of service authorization to members and care managers.
- Graduate from an Accredited School of Nursing or a Bachelor's degree and 4–6 years of related experience.
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
- NP - Nurse Practitioner - Current State's Nurse Licensure required.
