Key Responsibilities- Conduct comprehensive biopsychosocial assessments to identify emotional, social, and environmental strengths and problems related to patients' diagnoses, illnesses, treatments, and life situations.
- Develop and implement individualized treatment plans utilizing evidence-based clinical social work interventions, including crisis intervention, brief and long-term individual, marital, family, and group therapies.
- Screen, identify, diagnose, and treat mental health and substance abuse problems in patients and family members, utilizing DSM nomenclature.
- Collaborate with the triage team and triage RN to coordinate care and manage small caseloads of patients requiring bridge to another level of care.
- Provide education to patients and families on issues related to adaptation to their diagnoses, illnesses, treatments, and life situations.
- Participate in multidisciplinary healthcare teams, providing leadership in representing the clinical social work perspective and liaison with patients and families.
- Maintain a working knowledge of relevant medical and legal issues impacting patient care, including advance directives, child and elder abuse.
- Master's degree in social work from an accredited school of social work.
- Current licensure as a licensed clinical social worker (LCSW) by the NC Social Work Certification and Licensure Board.
- Two years of post-internship social work experience in a healthcare setting or social service agency.
- Strong assessment, interviewing, and counseling skills, with expertise in mental health and substance abuse areas.
- Ability to work effectively and autonomously in a self-directed role, with strong interpersonal and team-building skills.
