Job Summary

We are seeking a skilled Community Counseling Case Manager to join our team at Youth and Family Alternatives, Inc. This role involves providing part-time assistance in managing cases of truant and problematic youth and their families.

Key Responsibilities
  • Conduct assessments and prepare treatment plans under the supervision of the Team Supervisor and Program Director.
  • Monitor school attendance and offer school support as needed.
  • Provide case management services, including determining eligibility, intake, assessment, evaluation, and referral.
  • Schedule and conduct client visits at convenient locations and maintain contact with assigned schools.
  • Complete documentation related to case management services, including contact notes, assessments, and correspondence.
  • Conduct client and family support and education sessions, and serve as an advocate for clients.
  • Maintain up-to-date knowledge of laws, DCF, DJJ, and other agency guidelines.
  • Complete client follow-up according to established guidelines and handle crisis calls from clients and non-clients.
Requirements
  • Bachelor's Degree; with related experience in case management preferred.
  • Strong written and verbal communication skills, with the ability to work effectively in a team environment.
  • Proficiency in Microsoft tools and ability to learn new software as needed.
