Senior Community Care Nurse Practitioner Opportunity

Optum Home & Community Care is revolutionizing healthcare by uniting industry-leading solutions to create an integrated care model that addresses an individual's physical, mental, and social needs. As a Senior Community Care Nurse Practitioner, you will be part of a team that provides care to patients at home, in nursing homes, and assisted living facilities.

Key Responsibilities:

  • Conduct comprehensive assessments and manage members through acute medical situations or exacerbations of chronic conditions.
  • Ensure accurate and complete documentation, including ICD-10 conditions, and communicate with the interdisciplinary care team.
  • Provide patients and caregivers with counseling and education on advanced illness and end-of-life discussions.
  • Manage members during short-term sub-acute rehabilitation episodes and conduct advanced illness and advanced care planning conversations.

Requirements:

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Active and unrestricted APC license in the State of New Jersey or ability to attain by start date.
  • Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family Nurse Practitioner, Adult Nurse Practitioner, Gerontology Nurse Practitioner, Adult-Gerontology Acute Care Nurse Practitioner.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.

Preferred Qualifications:

  • 1+ years of clinical experience in a long-term care setting.
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of Geriatrics and Chronic Illness.
