Telephonic RN Case Manager Role Overview

Key Responsibilities:

As a Telephonic RN Case Manager, you will have the opportunity to make a significant impact on the lives of our members. Your key responsibilities will include:

  • Assessing the current health status of our members through outbound and inbound calls
  • Making referrals to outside sources to ensure our members receive the care they need
  • Identifying gaps or barriers in treatment plans and providing education to assist with self-management
  • Providing a complete continuum of quality care through close communication with our members via in-person or on-phone interaction
  • Serving as a resource to our members and supporting them with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels


Requirements:

To be successful in this role, you will need to have:

  • An undergraduate degree or equivalent experience
  • A current, unrestricted RN license in the state of employment
  • At least 3 years of experience in a hospital, acute care, or direct care setting
  • The ability to support a hybrid work schedule, including consumer-centric hours (11a-8p CST) two days a week
  • The ability to type and navigate a Windows-based environment


Preferred Qualifications:

We are also looking for candidates with:

  • A BSN
  • Certified Case Manager (CCM)
  • Case management experience
  • Experience or exposure to discharge planning
  • Experience in discharge planning or utilization review
  • Experience in a telephonic role
  • Experience in a leadership, account management, or client-facing role
  • Background in managed care
