Wellness Nurse Specialist

At Wickshire Senior Living Group, we are seeking a compassionate and skilled Wellness Nurse to join our team. As a key member of our healthcare team, you will be responsible for ensuring the health and wellbeing of our residents.
Job Summary:The Wellness Nurse will provide leadership, support, and assistance with resident care and services. This includes monitoring the health and wellbeing of residents, assisting in training, monitoring medication management, communicating with residents and families, coordinating support services, and maintaining clinical quality assurance in accordance with Federal, State, and local regulations and Wickshire's mission, policies, procedures, and standards.
Key Responsibilities:- Supervise Resident Assistants and assign shift duties when applicable.
- Fills in for shift call-outs.
- Assists residents with activities of daily living, including bathing, dressing, eating, toileting, hygiene, and mobility in accordance with the Resident Service Plan.
- Administers medications as required in compliance with state-specific regulations pertaining to medication technicians.
- Spot checks Medication, Treatment, and Service plan sign-off sheets for accuracy and complete documentation.
- Monitors the health, safety, and wellbeing of all residents, documents information pertaining to changes in residents' functional status and personal needs on the 24-hour shift report.
- Assists in the development and updating of each Resident Service Plan. Attends Service Plan meetings when applicable.
- Observes residents for changes in physical and/or behavioral conditions and timely communicates observations to the Health & Wellness Director.
- Contacts EMS (911) for resident emergencies and coordinates documents for transfer.
- Encourages resident participation in scheduled programs and activities.
- Conducts one-on-one or small group activities as assigned by supervisor in conjunction with the Wise & Well Program Director.
- Ensures compliance with Federal, State, and local regulations and Wickshire's mission, policies, procedures, and standards.
- Completes all mandatory compliance, HIPAA, and state and federal regulatory training as required.
- Maintains appropriate degree of confidentiality.
We are seeking a qualified candidate with:
- Graduate of a state-approved school of nursing.
- Current state license as a Registered or Practical/Vocational Nurse.
- Minimum of one year of experience in assisted living, hospice, home health, acute, or long-term care environment.
