Job Description:
As a Clinical Palliative Coordinator, you will play a vital role in our care team, working closely with patients, families, and healthcare providers to ensure comprehensive care. Your expertise and compassion will make a meaningful difference in the lives of those facing life-limiting illnesses.
Responsibilities:
- Evaluate patients' physical, emotional, and spiritual needs, and develop personalized care plans.
- Collaborate with the care team to ensure seamless care coordination and effective communication.
- Conduct regular assessments to identify areas for improvement and implement changes as needed.
Qualifications:
- Graduate of an accredited school of nursing with a current state license as a registered nurse.
- At least 2 years of experience in a clinical care setting, preferably in palliative care.
- Reliable transportation and ability to lift up to 25 pounds.
Salary and Benefits:
The estimated annual salary for this position is $90,000 - $115,000, depending on experience.
