We are seeking an experienced RN Case Manager to join our team at Baptist Hospice of Jackson, a part of LHC Group. This role offers a unique opportunity to make a meaningful difference in the lives of patients and their families.

Job Overview

The RN Case Manager will assume full nursing responsibility for delivering high-quality patient care, evaluating personal and professional performance, and making necessary changes to improve productivity and quality of care.

Responsibilities
  • Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Identify patients' physical, psychosocial, and environmental needs, reassessing as needed every 15 days.
  • Develop and coordinate individualized plans of care while maintaining accurate clinical notes on patients' conditions.
  • Collaborate with patients, families, attending physicians, and other members of the Interdisciplinary Group (IDG) to provide daily patient and family care.
Requirements

To be successful in this role, candidates must possess:

  • One year of clinical experience as a Registered Nurse (RN).
  • Current RN licensure in the state of practice.
  • Current CPR Certification.
  • A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
