About the Job:
We are seeking a highly skilled and dedicated Care Manager Specialist to join our team at DAY BROOK VILLAGE NURSING. This is an excellent opportunity for a compassionate and organized professional to make a meaningful difference in the lives of our residents.
Responsibilities:
- Deliver person-centered care to residents with Alzheimer's disease and related dementias, ensuring their physical, emotional, and social needs are met.
- Collaborate with healthcare professionals, family members, and caregivers to develop and implement individualized care plans that promote optimal quality of life.
- Provide ongoing support and education to residents and their families, promoting empowerment and independence.
- Manage complex care situations, advocating for residents' rights and dignity while maintaining confidentiality and respect for personal boundaries.
- Facilitate transitions and discharges, ensuring a seamless experience for residents and their families.
- Participate in interdisciplinary team meetings, contributing expertise and insights to enhance care coordination and outcomes.
- Stay up-to-date with best practices and research in gerontology, dementia care, and habilitative philosophy, applying this knowledge to improve resident care and experiences.
Requirements:
- 2+ years of experience in care management or a related field.
- Master's degree in a related field (e.g., social work, nursing, gerontology).
- Strong communication and interpersonal skills, with ability to build rapport with diverse stakeholders.
- Proficiency in electronic health records and care management software.
Salary: $80,000 - $105,000 per year, depending on experience.
