Care Coordinator Home Nurse Specialist

We are seeking a skilled and compassionate Registered Nurse to join our team as a Care Coordinator Home Nurse Specialist. In this role, you will be responsible for conducting initial and periodic client assessments, developing care plans, and ensuring the highest level of quality care for our clients.
Responsibilities- Conduct thorough intake assessments for new home care admissions to identify client needs and develop personalized care plans.
- Determine customer requirements and provide tailored service solutions to meet their needs.
- Collaborate with Care Coordinators to ensure seamless start-of-care experiences for clients.
- Supervise and support field staff to maintain high standards of care delivery.
- Perform caregiver skills assessments and in-home supervisory visits to monitor care quality.
- Active Maryland and District of Columbia RN license is mandatory.
- Excellent communication, interpersonal, and leadership skills to effectively collaborate with clients and staff.
- A minimum of 2 years' experience in nursing, preferably in a home care or gerontology setting.
- Competitive salary range: $80,000 - $110,000 per year (dependent on experience).

- Mileage reimbursement for on-duty travel.
