Monroe Plan for Medical Care is seeking a dedicated and compassionate Peer Specialist to join our team. As a Peer Specialist, you will play a vital role in supporting individuals who are experiencing homelessness and mental health challenges.
Job Summary:The Peer Specialist will provide community outreach and engagement services to individuals in need, working to connect them with essential resources and support. This role requires a strong understanding of mental health and substance abuse issues, as well as excellent communication and interpersonal skills.
Key Responsibilities:- Provide persistent and assertive outreach and engagement services to individuals in need, using strength-based approaches to build trust and rapport.
- Conduct comprehensive psychosocial assessments to identify the medical, psychiatric, housing, and other social needs of individuals.
- Develop and implement person-centered care plans in collaboration with individuals and their support networks.
- Work with regional partners to identify available housing and support individuals through the housing process.
- Collaborate with community providers to ensure individuals receive connected and coordinated care.
- Collect and report data to inform future care delivery and program development.
- Minimum High School Diploma or equivalent required.
- NYS Certified Peer Specialist preferred or completion of an internship or comparable training in peer advocacy.
- Experience in providing advocacy services to individuals with mental health and substance abuse issues.
- Knowledge of homeless resources, shelter systems, and transportation systems.
- Ability to develop, evaluate, implement, and modify treatment interventions to meet the needs of individuals.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- NYS driver's license and reliable transportation.
- Computer literacy and ability to pass computer documentation competency testing.
