Healthcare Navigation Specialist

About the Role: We are seeking a skilled Healthcare Navigation Specialist to join our team at i4 Search Group. This is a full-time, permanent position working remotely from Tennessee.
Job Summary: As a Healthcare Navigation Specialist, you will be responsible for providing exceptional care coordination services to Medicaid, Medicare Advantage, and/or Dual Eligible Special Needs Plan (DSNP) members.
Main Responsibilities:
- Conduct outreach to motivate, facilitate, and educate members about the benefits of programs.
- Assess and develop a care plan with each member to address their goals and needs.
- Evaluate individual member care needs and communicate medical information to healthcare professionals.
- Manage a caseload of members to ensure timely contact and expedient resolution of issues.
- Facilitate coordination of care with providers and schedule appointments as needed.
- Identify and help address needs related to Social Determinants of Health.
- Coordinate and complete correspondence according to established workflows.
- Thoroughly document actions taken in a care management platform.
Requirements:
- 2+ years of experience as a care coordinator, case manager, community health worker, pharmacy technician, or social worker.
- Fluency in English; Spanish fluency preferred.
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) preferred.
- Dedicated home office for remote work.
- Must reside in Tennessee.
- Experience working with Medicaid and/or Dual Eligible patients.
Preferred Skills:
- Strong interpersonal, communication (both verbal and written), and problem-solving skills.
- Experience working with customers over the phone and by text message.
- Experience with Apple computers (MacBook).
- Experience documenting case notes in a care management or electronic health record platform.
- Experience with motivational interviewing.
