Job Summary

The Medical Social Worker III provides comprehensive assessments, coordination, and implementation of professional social services to patients and their families, addressing social, emotional, and economic difficulties related to medical problems or predisposing to illness.

Key Responsibilities
  • Uphold Kaiser Permanente's policies and procedures, principles of responsibilities, and applicable state, federal, and local laws.
  • Provide quality services by assessing patient/family system needs, developing and implementing treatment plans in accordance with departmental and medical center policies.
  • Perform bio-psychosocial assessments, provide crisis intervention, counseling, information/referral, and consultation to physicians and healthcare teams.
  • Develop culturally sensitive assessments, provide counseling services, and perform patient and family advocacy services within KP and the community.
  • Provide patient education on psycho-social adjustments to medical illness, individually, in classes, or groups.
  • Take responsibility for implementation of social work knowledge and values, standards of practice, and enhancement of professional skills.
Interdisciplinary Functions
  • Function as an effective team member, consultant, and liaison to medical, nursing, ancillary staff, and community agencies.
  • Demonstrate an interdisciplinary approach, competent follow-up, and communication of accurate information to team members.
  • Assume responsibility for patient, staff, and system problems as indicated by requests for involvement.
Professional Conduct
  • Demonstrate professional behavior and interventions consistent with departmental policies and procedures, JCAHO, Title 22, and NCQA legal mandates, professional standards, NASW, ABECSW, and other Medical Center Guidelines.
  • Utilize supervision and consultation appropriately as indicated by regular attendance at supervisory/consultation sessions, identifying and discussing problem cases/situations with supervisor, and seeking consultation with peers.
  • Contribute and share community resources with colleagues.
  • Represent the Department in appropriate medical center and/or community committees.
Requirements
  • Masters Degree in Social Work issued by a school accredited by the Counsel of Social Work Education.
  • Licensed Clinical Social Worker (California).
  • One (1) year post-masters experience in Medical Social Work required.
  • BLS Card required.
Preferred Qualifications
  • Prior ER experience preferred.
