Overview

We are a San Diego-based hospice agency seeking a compassionate and dependable Hospice Social Worker to provide emotional support and address the psychosocial needs of patients and their families. Our team is dedicated to delivering exceptional care and promoting a work-life balance for our employees.

Responsibilities
  • Provide in-home and facility visits to assess patient psychosocial status and needs
  • Develop and implement MSW care plans and institute appropriate interventions
  • Collaborate with community resources and third-party providers as needed
  • Prepare patients, families, and caregivers for end-of-life processes from a social work perspective
  • Communicate effectively with referral partners and provide educational inservices as necessary
Requirements
  • Masters of Social Work degree
  • Experience working with death and dying, providing support to patients, families, and caregivers (preferred)
  • Licensed driver with automobile insured in accordance with state requirements
  • Current CPR Certification meeting American Heart Association BLS Guidelines
