About the PositionWe are seeking a skilled and compassionate Nurse Practitioner to join our team as a PRN/Part Time provider. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work with a diverse range of patients and families, providing comprehensive health assessments and developing personalized care plans to address their unique needs.
As a PRN/Part Time provider, you will have the flexibility to choose your own schedule and work at your own pace. You will also have the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth visits.
Responsibilities- Conduct comprehensive health assessments of patients in their homes or other community settings
- Develop and implement personalized care plans to address the unique needs of each patient
- Collaborate with other healthcare professionals, including physicians, case managers, and social workers, to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy behaviors and improve health outcomes
- Document all patient interactions and care plans in accordance with Matrix Medical Network's policies and procedures
- Master's degree in Nursing or related field
- Current RN and NP licensure in state of practice
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
- Excellent verbal and written communication skills

We offer a competitive compensation package, including a hourly rate of $90.00 per completed assessment. We also offer a mileage reimbursement plan and opportunities for professional growth and development.
